Application for Assistance

The Application for Assistance is a .PDF fillable form. You may download the file or open it online, depending on your browser settings. Once the form is open, type the requested information into all applicable fields and then follow the Submitting the Application instructions at the end of the form.

If you open the form in your browser and fill it out, use the Print to PDF option to save your application to your computer or other device. If you choose to print a hard copy, BE SURE TO SAVE THE FORM BEFORE PRINTING. Printing a hard-copy of the Application from some browsers does not prompt the user to save the form after printing and all information in the form will be cleared.
